I also have a Thomas J. McCarthy as "Witness" in Fallen, although it looks like a different person according to imdb. http://www.imdb.com/name/nm0565339/ Hope all those 'credited as Tom' don't make this an effort in impossibility.

1) I have Syriana as Tom McCarthy (just checked the Blu-ray)

2) Baby Mama is also Tom

3) I have two Tom McCarthy's in my DB.  One gets almost all the credits you have listed including the crew credits.  He wrote/directed The Station Agent and The Visitor in addition to the acting.  He was born in 1966 (WikiPedia)

I also have a Tom McCarthy in Blow Out with a BY of 1936 (IMD* only source for that but the other Tom would've been too young to play a policeman in Blow Out).

Just to make it more fun both of them are have 'J' as the middle initial.

This seems to muddy things up a bit. The Tom McCarthy in Blow Out also goes by the name Thomas J. McCarthy over at IMDB, but Tom McCarthy appears to be his common name if IMDB is to be trusted. It doesn't look like there's an accepted BY for either actor in the DB. To make matters worse both Tom McCarthies acted in Season 5 of The Wire, both credited as Tom McCarthy.

Using the Credit Lookup tool, there also appears to be a sound editor by the name of Tom McCarthy.
