In the splatter/gore film The Dead Next Door, there is a visual credit in the end credits as this:



Jon Killough definitely receives a credit as Crazy Grady Zombie, but would you also credit "A Neat Fake Head"? It obviously refers only to the Make-up Mask effect (almost a Make-up Crew reference really) and not an actual actor.

I'd be interested in hearing some opinions on how to enter it correctly.

For "joke" credits like this I tend to use the "credited as" to show what's screen

So in this case I would put Jon//Killough [Jon Killough and a Neat Fake Head] as "Crazy Grady Zombie"

Personally I wouldn't credit the head, but then I have the advantage of being able to see things in shades of gray.

I wouldn't credit the head either.

As to your what you should do, maybe this will help...if it were a credit of, "John Smith and Joe Smith as Crazy Grady Zombie," would you use the credited as or enter two individual credits?  If your answer is the former, use credited as.  If you answer is the latter, then 'Jon Killough as Crazy Grady Zombie'.

Thanks for the input. I really wouldn't even enter it either and was leaning towards that, except it's already added in the existing profile.

I'll contribute the head's removal  and see what happens...

But aren't this in fact two "actors" sharing the same role??
So the correct entry would probably be
Group divider named: "Crazy Grady Zombie"
Jon Killough
A Neat Fake Head //
End Group divider

Just kidding of course: Don't enter it

Because it would mess up the CLT results for Jon Killough and is an incorrect interpretation of the credits.
The smaller "and" clearly indicates that we are talking about two "actors" here that share one role.
So if we'd enter "A Neat Fake Head" at all, it would be below a group divider named "Crazy Grady Zombie" together with Jon Killough as the second member of this group.

But as said before: Since this "Neat Fake Head" is directly comparable to "Special Trained Mountain Llamas", it shouldn't be entered at all.
It all seems so stupid, it makes me want to give up!

It's not, I checked before I posted. 
And I still think it's the best way of dealing with these odd credits. Personally I think removing these quirky little additions makes the profile a little dry, whereas including them mimics the humour found within the film itself.
In terms of contributing to the online I would have no objections either for or against adding the credit, but now that it's on the profile I don't see the harm in just leaving it. It's not bad data.
