almost looks like the old Laser disc logo for surround sound ( with no center speaker) as that was what surround sound was in the 80's ...  Left Right/ Left Right ..

Most likely the cover is wrong. That's the arrangement for quadrophonic, which as far as I know, was only ever used for music.

Four channel movies have always been like the graphic in DVD Profile.

I've just checked the one 4.0 DVD I have (Buffy the Vampire Slayer film) is indeed as per the DVDP representation.
