In DVDprofiler itself go to tools, CCVPlugin, Import Awards then go to View, Windows, and then awards. A window should then appear that you can place any where.

While Profiler is closed, you must run the setup of the plugin. How to go from there, is described in the included readme file.

The readme is included with the plugin, and not the data. I should have opened after setup was finished (except if you unticked the checkbox ).
You can also find it in your program folder\DVD Profiler\Plugins\CCVPlugin

Here the instructions from the readme:
First Steps:
1) After installation of Plugin open DVD Profiler 3
2) If you already have an existing Cinematic Collection Viewer database
  you can select it with
    "Tools->CCVPlugin->Select database..."
  otherwise you can create a new database with
    "Tools->CCVPlugin->Create database..."
    and enter a name and path for the database. You can put it anywhere you like.
3) If no award data is inside the database, download the award files from my website
  and extract the Zip-files.
  Then import them with "Tools->CCVPlugin->Import Award File(s)..."
4) Open Award window with "View->Windows->Awards" and position it anywhere you like.
  It can be integrated into the Profiler's layout
