Working on Dark Shadows, at episodes 349 and 350 there is a credit "Special Make-up by Dick Smith."  There are no other make-up credits on these episodes.  Smith was not the usually credited makeup artist (who was Vincent Loscalzo).  On the bonus interview, Smith talks about being called in to do the special age makeup for Barnabas when he suddenly become 200 years old.  "Special Make-up" is not exactly one of the listed credits in the Profiler, though it's pretty close to the "Special Make-up Effects by" listed for "Make-up Effects." So, how would you credit Smith, if you would credit him at all?

I would credit him as 'Make-up Effects'.
No dictator, no invader can hold an imprisoned population by force of arms forever.
There is no greater power in the universe than the need for freedom.
Against this power, governments and tyrants and armies cannot stand.
The Centauri learned this lesson once.
We will teach it to them again.
Though it take a thousand years, we will be free.

Make-Up Effects.
This should really be on the grid so nobody can say "it's a equivilant and doesn't belong", it's just an abbriviated Make-up Effects credit in my eyes. Special Make-up Effects is good, this is just that with the "Effects" word. If the "Special" was gone it would be no question at all as the rules read.

Make-up Effects definitely. I'd add him to the profile without much of a second thought.

I would enter him as Make-up Artist if there's no credit for Make-up Effects, else I would enter him as Make-up Effects. For example: Casino (1995) Make-up Effects by KNB FX Group and Special Make-up by Ken Diaz.

OK, it's going in as Make-up Effects.  Thanks for the input.

Is 'Special Make-up by' a direct translation of 'Make-up Effects' or 'Special Make-up Effects by'? IMO it's not! It may be a functional equivalent though. But those are not allowed by the rules. Invelos could you give a clarification on this topic? Do we need a rule change?
