Just what the title says.  This happened a few months ago (I think when I upgraded to 3.1).  However I'm just getting around to asking about this.  I've since updated to 3.1.1 build 1171 and still seem to have the issue.  I tried searching through support and the forums but no luck.  I've also uninstalled and reinstalled the software to no avail.

Examples of the issue are circled in red in the following screen shot.

Probably not the solution but i'll toss it out there.  Is your Display DPI setting something other than the default (aka Normal)

(Control Panel|Display Properties|Settings|Advanced|General|DPI Setting)

I've had problems with certain plug-ins that had buttons that were completely missing if the DPI was other than Normal.  Your problem differs from mine but it might be a way to investigate.

Nope, DPI settings are set at Normal size at 96 DPI.
**EDIT** It's also not my screen resolution as I've tried everything from 800x600 to 1280x1024.

Maybe you should update your graphic device driver. After this try to set screen resolution and other graphic related settings to "default".
You don't use Windows Blinds or some other desktop "enhancements"?
If you start your computer in "safe mode" do you experience the same problem? There can be many other causes for this weird behaviour! Good luck!

don't forget to also try Load Layout - reload  ( under view along top task bar) ..
