I own The Omen Collection and noticed the cover doesn't match anything in the database even though the disc ID does match one of those. Here's the thing, that one doesn't match the release date either. How should I submit this? I withdrew my image contribution because I'm sure it would conflict with those who do own that version (unless whoever submitted that one with the disc ID didn't scan the new cover...)

As far as I know, The Omen Collection was released three times in different packaging, with either some changes or without, repeating Disc IDs of one or the other sequel in one or the other of the releases. I hope, I could express myself. 

Whatsoever, I think you should only amend the profile locally, but NOT contribute the same Disc ID with another release date and/or another cover.

If it's the region 1, then Omen 2-4 in the recent box set are the same discs as those from the original set. However, in the original set, #4 wasn't separately released, so it had no UPC of its own. Therefore in the new set, 2 and 3 can be listed under disc ID with no conflict, but #4's disc ID belongs to the original set's profile. All you need to do is change the scans and release date.
