Details maybe to vague regarding to what you submited or changed in the profile.

Introduction

Thank you for your interest in contributing to the DVD Profiler main database!
"The authoritative source for information submitted should be the DVD itself.  Please don't submit content from a third party database, and always verify the specifications printed on the cover.  In both cases, errors abound, so always verify the information directly from the DVD whenever possible."

it my help if you give some more details on what profile your working. Some one more experienced may offer you assistance.
Sources for one or more of the changes and/or additions were not submitted. Please include the sources for your changes in the contribution notes, especially for cast and crew additions.

it means : "please state where you found the crew/cast info"

what it doesn't say but is implied : only crew/cast info from the on-screen film credits are acceptable.

there is more to it:

Credits

"If an actor is credited by name but does not have an associated role, you may use another source to identify the role. In each case, list Actor’s names and roles (when given) exactly as they are in the credits and in exactly the same order credited."
Sources for one or more of the changes and/or additions were not submitted. Please include the sources for your changes in the contribution notes, especially for cast and crew additions.

Did you really put your brain as a source in the contribution notes?    If so, the screeners usually don't do well with those kinds of things.  They need specific sources, and your brain doesn't count.

Sometimes the screeners can be 'finicky'.  Other times it is just an honest mistake.  They go through quite a few contributions, each and every day.  If something looks odd, it isn't given the benefit of the doubt.

I would just resubmit with notes that are a little bit clearer.

Could it be that your contribution notes have been read by the screeners as "credits from the dvd cover"? Because there have been cases where what the dvd cover says is different from what's in the actual film's opening and end credits. And it's the latter that should be contributed, as sxilderik mentioned.
If you used the term "dvd credits" rather than "the film's (opening/end) credits", that could be ambivalent.

Your contribution was declined because the contribution notes did not contain any sources. Your contribution notes were "forgot to scan dvd cover before submitting originally "

When a contribution comes to the screeners, it only shows the latest set of comments, the whole history of contribution notes do not show cumulatively on the screen.

I previewed your resubmission and it should be fine because it states that you got the listing from the credits of the DVD.

This is a good thing to remember.  When I re-contribute a profile because I forgot something, I'll copy the original notes after stating what I corrected/amended.

Thanks for the explanation Gerri.  I suspected each contribution stood on it's own.  It makes sense.  It would take forever to process if you had to read all the previous notes as well as the current ones.

I'm wondering why my new scans have been declined.
I received 3 yesvotes and wrote Improved Coverscans with better colors

1) The wrong button might have been pressed by the screeners, it happens.
2) The screeners didn't see a source for the art work listed like: "my DVD cover"
3) The screeners didn't like the scan or think the improvement was enough.  Our votes are only recommendations.  Screeners have supreme authority.

Generally you scans do look better though there are some spots (under the 'C' in Carrey and on Jim's eyebrow) that i'd clean up.

You can send a message to Gerri Cole and see if she can explain the problem or resubmit and try again.
Bad movie?  You're soaking in it!

It is very important with images to be specific about what your changes are if you are submitting a full size image and there is already one there. You should point out specific differences and call them out. The scans must be significantly different in order to be accepted.
